Trainee Forms
This page provides access to forms required throughout General Surgery training. Trainees are responsible for completing and submitting the appropriate forms at the correct stage of training, in accordance with the GSET and SET Regulations.
Forms are reviewed and updated from time to time. Please ensure you are using the most current version available on this page.
Using Trainee Forms
Before completing any form, Trainees should:
- Confirm the form is required for their current training pathway (SET or GSET)
- Read any accompanying instructions carefully
- Complete all sections accurately and in full
- Obtain required supervisor or trainer sign‑off
- Submit the form via SOLA or as directed
Incomplete or late submissions may affect training assessments, progression, or eligibility for certification.
Assessment and Progression Forms
These forms support the assessment of Trainee performance and progression during training rotations.
- GSET End‑of‑Term / Mid‑Term In‑Training Assessment
Used to assess GSET Trainees’ performance during each training rotation. The form captures feedback across clinical competence, professional behaviours, and overall progress, and must be completed and submitted within the relevant assessment timeframe.

- SET End‑of‑Term / Mid‑Term In‑Training Assessment
Used for Trainees undertaking the SET pathway. This form records performance and progression during each accredited training term and contributes to overall training outcomes..

Learning and Development Forms
- Personal Development & Learning Plan (PDLP) – GSET Trainees
The PDLP supports reflective practice and structured learning. GSET Trainees use this form to identify learning needs, set goals, and plan development activities in collaboration with their Supervisor or Trainer.

- Fellowship Examination Preparation Record
This form is used to document preparation activities for the Fellowship Examination. It may be required to demonstrate readiness and preparedness in advance of applying to sit the examination.

Research Forms
These forms relate to research activity undertaken during training and must be completed as required.
- Trainee Request Form – Prior Approval of Research Topic
This form is required before commencing a research project. Trainees must obtain appropriate approval to ensure the project meets training and programme requirements

Used to report on the status of an approved research project. Trainees are required to provide updates in line with programme expectations and timelines.

General Request Forms
- Trainee Request Form
This form is used for a range of trainee‑initiated requests. Trainees should ensure all relevant details and supporting documentation are included to allow requests to be considered appropriately.

Submission and Record‑Keeping
Unless otherwise specified:
- Completed forms should be uploaded via SOLA (Surgical Online Logbook and Assessments)
- Forms must be submitted within required timeframes
- Trainees should retain copies of all submitted documentation for their own records
